DAY PROGRAMS
Day Activity Programs are for loved ones who can self-care and are not at risk for wandering. They do not need one-on-one caregivers. Day activity environments are socially enriching and a way for a caregiver to engage with their loved one and others. Fee Based.
Adult Care Day Programs are specialized memory day programs where loved ones are delivered to a secure unit and stay under the care of trained caregivers. They are to be picked up at the close of the program.
Cheer of DE
“Enjoy daily activities such as exercise programs, Tai Chi, Yoga, Zumba, Wii™ games, Mahjong, support groups, craft classes, health seminars, concerts, games, educational and cultural trips, bingo, etc.
All centers offer opportunities for CHEER members to socialize and exercise their brains and bodies.” Memory Cafe is offered at some locations.
Annual Membership
Inexpensive breakfast and hot lunches served, prepared meals available for purchase to take home.

An independent Adult Activity Center for individuals 50 years and older. Participants should be able to maintain their independence and take care of themselves as care-giving services are not available.

ADULT CARE DAY PROGRAMS
Memory Care Day Programs provide a secured unit where Loved Ones are under the care of trained providers. A nurse, caregivers and activity coordinators are on site. You do not remain onsite with your loved one. Fee based.
Sand Hill Day Program

The Sand Hill Adult Day Program is designed to engage members socially, intellectually and creatively, based on their individual needs and functional ability.” - Per Website
Program is an adult program in a secure center under care. Caregivers drop their loved one off and leave them with the program staff.
Participants can attend from one to up to five days a week.

“The Memory Care Program will include modifications such as secured doors and special program areas designed to better serve families. Comprehensive adult day services on-site in Milford. Also included: home health services, inpatient, transportation, lab tests, primary care, specialty physician care, social services, rehab and durable medical.

The Program of All-Inclusive Care for the Elderly (PACE) gives participants the ability to stay in their home while receiving essential services and maintaining independence and physical and emotional well-being.
PACE Your LIFE provides comprehensive medical and social services to adults 55 and older that reside in their home, requiring assistance and mostly to individuals who are eligible for Medicare and/or Medicaid benefits. A team of healthcare professionals provide PACE participants with personalized medical care.
